Your Information. Your Rights. Our Responsibilities.
This notice describes how medical information about you may be used and disclosed, how you can get access to this information, and your rights regarding your health information. Please review it carefully.
This Notice of Privacy Practices applies to Integrated Provider Group of Minnesota PLLC (“IPGMN,” “we,” “us,” or “our”) and the healthcare professionals, employees, and other workforce members providing services through IPGMN clinic locations.
Your Rights
When it comes to your health information, you have certain rights. This section explains those rights and some of our responsibilities to help you exercise them.
Get an electronic or paper copy of your health record
You may ask to see or obtain an electronic or paper copy of your medical record and other health information we maintain about you. We will generally provide access, a copy, or a summary of your health information within the time required by applicable law.
Minnesota law also provides patients with rights to access their health records. We will provide access and copies in accordance with applicable federal and Minnesota law. We may charge a fee for copies when permitted by law, limited to the amount permitted under applicable federal and Minnesota law.
Contact us using the information at the end of this notice to request access to your health records.
Ask us to correct your health record
If you believe health information we have about you is incorrect or incomplete, you may ask us to amend or correct the information. We may deny your request in certain circumstances permitted by law. If we deny your request, we will explain the reason to you in writing and tell you about any additional rights you may have.
Request confidential communications
You may ask us to communicate with you about your health information in a particular way or at a particular location — for example, to call you only at a particular telephone number, send correspondence to a different mailing address, or use another reasonable method of contacting you. We will accommodate reasonable requests as required by law.
Ask us to limit what we use or share
You may ask us not to use or disclose certain health information for treatment, payment, or healthcare operations. We are generally not required to agree to every requested restriction. If we agree to a restriction, however, we will comply with it except in certain situations permitted by law, such as when information is needed for emergency treatment.
Services you pay for in full
If you pay for a healthcare service or item entirely out of pocket, you may ask us not to disclose information about that service or item to your health plan for purposes of payment or healthcare operations. We will honor that request unless disclosure is otherwise required by law.
Get a list of certain disclosures
You may ask for an accounting of certain disclosures of your health information. This accounting generally includes certain disclosures made during the six years before the date of your request. It does not include every disclosure — for example, disclosures for treatment, payment, and healthcare operations generally are not included.
We will provide one accounting during a 12-month period without charge. We may charge a reasonable, cost-based fee for additional accountings during the same period when permitted by law, and we will tell you the cost before proceeding.
Get a copy of this Notice
You may request a paper copy of this Notice of Privacy Practices at any time, even if you previously agreed to receive it electronically. A current copy is also available on our website.
Choose someone to act for you
If you have given another person legal authority to act for you, such as through a healthcare power of attorney or legal guardianship, that person may exercise your privacy rights and make choices about your health information as permitted by law. We may require documentation confirming the person’s authority before acting on a request.

Your Choices
For certain health information, you have choices about what we disclose. If you have a clear preference regarding the situations described below, tell us what you want us to do. We will follow your instructions when required by law.
Family members, friends, and others involved in your care
Unless you object, we may share health information with a family member, close friend, caregiver, or another person involved in your healthcare or payment for your healthcare.
If you are unable to tell us your preference, such as during an emergency or when you are unconscious, we may disclose relevant information if we reasonably believe doing so is in your best interest and the disclosure is permitted by law. We may also disclose information when necessary to prevent or lessen a serious and imminent threat to health or safety when permitted by law.
Disaster relief
We may disclose limited information to an organization assisting in a disaster-relief effort when permitted by law so that family members or others responsible for your care can be notified about your location, condition, or status.
Uses that generally require your written authorization
We will generally obtain your written authorization before using or disclosing your protected health information for marketing purposes when authorization is required by law, the sale of protected health information, most uses or disclosures of psychotherapy notes (if we maintain such records), and other purposes not described in this notice when authorization is required by law.
If you authorize us to use or disclose your health information, you may revoke that authorization in writing at any time, except to the extent that we have already acted in reliance on it or another exception applies.
Fundraising
IPGMN does not currently use protected health information for fundraising communications.
How We Typically Use or Share Your Health Information
HIPAA and other laws allow us to use and disclose health information for certain purposes without obtaining a separate authorization each time. Minnesota law may provide additional protections for health records; when Minnesota law provides greater privacy protection than federal law, we will follow the applicable Minnesota requirements.
Treat you
We may use your health information and disclose it to healthcare professionals involved in your treatment. For example, a chiropractor treating you may share relevant information with another healthcare provider involved in evaluating or treating your condition, including physicians, chiropractors, physical therapists, imaging facilities, laboratories, pharmacies, or other healthcare providers involved in your care when permitted by applicable law.
Run our healthcare practice
We may use and disclose health information to operate our practice, improve the quality of care we provide, and manage our healthcare services — including reviewing the quality of care provided by our clinicians, training staff, conducting internal audits, managing clinic operations, coordinating services, evaluating provider performance, maintaining and improving our healthcare systems, and contacting you when necessary regarding your care or our operations.
Bill for your services
We may use and disclose health information to bill for healthcare services and obtain payment from health plans or other responsible parties — for example, providing information about your diagnosis, treatment, or services to your health insurance company so it can determine coverage and process payment. This may include eligibility verification, prior authorization, claims submission, claims appeals, coordination of benefits, utilization review, and collection of amounts owed for healthcare services, all in accordance with applicable federal and Minnesota law.
Other Ways We May Use or Share Your Health Information
We may be permitted or required to use or disclose your health information for other purposes, subject to legal requirements and limitations, including:
- Public health and safety — preventing or controlling disease, injury, or disability; reporting certain injuries or conditions; reporting suspected abuse, neglect, or domestic violence; reporting adverse reactions to medications or medical products; assisting with product recalls; preventing or reducing a serious threat to a person’s health or safety
- Health oversight activities — audits, investigations, inspections, licensure or disciplinary proceedings, government healthcare-program oversight, and other activities necessary for oversight of the healthcare system
- Research — when permitted by applicable law and when required privacy safeguards have been satisfied; your written authorization may be required in some circumstances
- Comply with the law — we will disclose health information when federal or state law requires us to do so
- Workers’ compensation — as authorized or required for workers’ compensation programs or similar programs established by law
- Law enforcement — when specifically authorized or required by applicable law, limited to what is permitted or required
- Medical examiners, coroners, and funeral directors — when permitted by law and necessary for them to perform their duties
- Organ and tissue donation — to organizations involved in organ, eye, or tissue donation and transplantation, when applicable and permitted by law
- Government functions — military and veterans’ activities, national security and intelligence activities, protective services, and correctional institutions and law-enforcement custodial situations, as permitted by law
- Lawsuits, administrative proceedings, and legal actions — in response to a court or administrative order or in connection with subpoenas, discovery requests, or other lawful processes when the requirements of applicable federal and Minnesota law have been satisfied
Minnesota Health Records Protections
IPGMN is located in Minnesota and is subject to applicable Minnesota health-record privacy laws in addition to HIPAA. Minnesota law generally requires a patient’s signed and dated consent before a provider releases a patient’s health records unless the disclosure is specifically authorized by law. Minnesota law also provides patients with rights to access their health records and obtain copies of those records.
When Minnesota law provides greater privacy protection or more stringent requirements than HIPAA, IPGMN will follow the applicable Minnesota requirements. Certain categories of health information may also receive additional protections under federal or Minnesota law.
Substance Use Disorder Records
Federal law provides additional confidentiality protections for certain substance use disorder treatment records governed by 42 CFR Part 2. To the extent IPGMN creates, receives, or maintains records that are protected by 42 CFR Part 2, those records will receive the protections required by that law.
Information from a record protected by 42 CFR Part 2 generally may not be used or disclosed in a civil, criminal, administrative, or legislative investigation or proceeding against you unless the requirements of federal law are satisfied, including your written consent, or an appropriate court order and subpoena or other legally required process. Part 2 information also may be subject to additional restrictions regarding redisclosure, fundraising, and other uses.
Our Responsibilities
IPGMN is required by law to maintain the privacy and security of your protected health information, follow the privacy practices described in this notice, provide you with a copy of this notice, notify you following a breach of unsecured protected health information when notification is required by law, honor the privacy rights available to you under applicable law, and use or disclose only the information appropriate for the permitted purpose, when required by law.
We will not use or disclose your protected health information for purposes other than those described in this notice unless you provide written authorization or the use or disclosure is otherwise permitted or required by law. If you give us written authorization, you may revoke that authorization in writing at any time, subject to limitations permitted by law.
Sale of Health Information
IPGMN does not sell your protected health information. We will not sell protected health information without an authorization that satisfies applicable legal requirements.
Patient Portal and Electronic Communications
IPGMN may provide patients with access to health information or healthcare communications through a secure patient portal or other approved electronic system. We encourage patients to use secure communication methods when discussing sensitive medical information.
General website contact forms, ordinary email, text messaging, and other nonsecure communication methods may not be appropriate for sensitive healthcare information. If you have questions about your treatment or wish to communicate sensitive medical information, please contact your clinic or use the secure patient portal when available.
Changes to This Notice
We may change the terms of this Notice of Privacy Practices. If we make changes, the revised notice may apply to all protected health information we maintain, including information created or received before the change.
The current Notice of Privacy Practices will be available upon request, available at our clinic locations, posted prominently where required, and available on our website. The effective date shown at the beginning of the notice indicates when the current version became effective.
