Integrated
/ˈin-tə-ˌgrā-təd/
adjective
- 1. Combined or coordinated into a unified whole.
- 2. (in healthcare)care that is delivered as a connected plan across providers, services, and records — rather than as a series of disconnected appointments.
That word is in our name on purpose. Here’s what it looks like in your care.
Integrated care at IPGMN
We’re part of your medical team, not separate from it.
IPGMN serves patients from every major Twin Cities health system. Our formal partnership in the Allina Integrated Medical (AIM) Network — along with our providers’ 25+ years working inside those systems — means we stand inside of your care, not next to it. For most of our patients, that shows up in three concrete ways.
Most patients arrive by referral.
Some come to us through a primary care doctor or specialist who trusts us with their patient’s care. Others come through a friend or family member already in our care. You don’t need a referral to see us, but trust like that — earned with doctors and patients alike — is the foundation of how integrated care begins.
We start with the full picture.
Through our AIM Network membership, we can access your imaging, test results, and notes from your other providers — using the same electronic medical-records system your primary care doctor uses. We arrive informed, not guessing.
Your care team stays in the loop.
Our treatment notes appear in your shared medical record, so your referring doctor or specialist can see what we’re doing in your care. When a case needs a real conversation, we reach out directly.
This isn’t a new way of working for our team. Our providers spent more than 25 years inside Minnesota’s largest health systems — Allina Health, Park Nicollet, Hennepin Healthcare, and HealthPartners — working side-by-side with primary care doctors, physical therapists, and specialists. That medical-team background is the reason we operate the way we do today, and it’s what made the AIM Network partnership a natural fit.
